VFC/VFAAR Re-enrollment Begins Today!
The Philadelphia Vaccines for Children (VFC)/Vaccines for Adults at Risk (VFAAR) annual re-enrollment period is now open!
Sites enrolled in VFC and/or VFAAR must submit a re-enrollment form by February 28, 2025 to continue participating in these programs.
Follow these 3 steps for a smooth re-enrollment!
1. Complete this survey.
Your answers will help us better support you throughout the next year.
A submission is required to complete re-enrollment for your site.
2. Update Clinic Tools
Update the information indicated in the Clinic Tools, Clinic Information module for your site. Use these job aids to guide you through making the necessary changes to the clinic tools page:
- Staff Changes Job Aid
- The primary and back-up vaccine coordinators must document completion of the CDC’s You Call the Shots training.
- VFC: If you have taken this training before, you may complete the VFC Refresher Test instead.
- VFAAR: If you have taken this training before, you may complete the Storage and Handling Refresher Test instead.
- Add the training(s) to your PhilaVax staff page by following these instructions.
- The primary and back-up vaccine coordinators must document completion of the CDC’s You Call the Shots training.
- Address or Delivery Hours Update Job Aid
3. Use these job aids to guide you through the enrollment form step by step:
- Enrollment Job Aid
- When completing the enrollment, you will be asked to provide updated patient population data. This can be found by running an insurance report using your EHR or billing software.
- Medical Director Signature Job Aid
Do you have access to PhilaVax?
The vaccine coordinator and medical director must log into the PhilaVax IIS to access, complete, and sign the electronic re-enrollment form. If you have not completed the 2025 User Confidentiality Agreement to renew your account or do not have a PhilaVax user account, use the link below to complete it now.
