01 May 2026 Action Required: VFC/VFAAR Re-Enrollment Due June 1
The Philadelphia Vaccines for Children (VFC) and the Vaccines for Adults at Risk (VFAAR) annual re-enrollment period is now open!
To continue participating, all clinics must submit required forms by June 1, 2026. Clinics enrolled in both programs must complete separate re-enrollments for each program.
Complete re-enrollment in 3 steps:
1. Submit the required survey
This submission is required to complete re-enrollment and helps us better support your clinic. Click the button below to access the survey.
2. Update your Clinic Tools information
Update the information indicated in the ‘Clinic Tools, Clinic Information’ module for your clinic. Use these guides to make the necessary changes to the Clinic Tools page:
- Staff changes: Staff Changes Job Aid
- Vaccine coordinator trainings: The primary and back-up vaccine coordinators must document completion of the CDC’s You Call the Shots training.
- VFC: If you have taken this training before, you may complete the VFC Refresher Test instead.
- VFAAR only: If you have taken this training before, you may complete the Storage and Handling Refresher Test instead.
- Add the training(s) to your PhilaVax staff page by following these instructions.

3. Complete the enrollment form in PhilaVax
- Be prepared to:
- Enter updated patient population data.
- Use this guide: Enrollment Guide
- When completing the enrollment, you will be asked to provide updated patient population data. This can be found by running an insurance report using your EHR or billing software.
- Obtain medical director signature.

- Enter updated patient population data.
Do you have access to PhilaVax?
The vaccine coordinators and medical director must log into the PhilaVax IIS to access, complete, and sign the electronic re-enrollment form.
- Confirm that you have access to PhilaVax by navigating to the PhilaVax website.
- If you are having trouble logging in, contact our support team at philavax@phila.gov.
- Fill out the PhilaVax User Agreement for 2026 if you haven’t already.
