The Philadelphia Immunization Program offers free print materials for Philadelphia based pediatric, family medicine, school based, pharmacy and internal medicine providers who promote or administer vaccination. Promote immunizations at your office, pharmacy or practice with posters, handouts, brochures, and stickers.

1. Pediatric Immunization Materials

Please use this form to request immunization materials from the Philadelphia Immunization Program. Any Philadelphia County based provider, supplying or promoting pediatric vaccination may request the following materials:

  • Stickers
  • Postcards
  • Posters
  • Storage and Handling materials
  • Laminated Immunization Schedules
  • Disease Fact Sheets
  • Certificate of Immunization
  • Medical Plan
  • Bookmarks
  • Activity Books
  • Growth Chart

2. Adult Immunization Materials

Please use this form to request adult immunization materials from the Philadelphia Immunization Program. Any Philadelphia County based provider, supplying or promoting adult vaccination may request the following materials:
