Read on for more information, and complete the interest form below by July 14th!<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The&nbsp;<strong>Bridge Program<\/strong>&nbsp;is a COVID-19 vaccine program for uninsured and underinsured adults 19 years of age and older in Philadelphia county.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The program will only offer COVID-19 vaccines for uninsured and underinsured adults, 19 and older.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Sites that wish to enroll in the Bridge Program will need to complete an enrollment form. This includes sites that are already enrolled in the COVID-19 program.<\/strong><\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\"><strong>Provider Eligibility<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Providers already seeing or are willing to see uninsured or underinsured adults, 19 years of age and older.<\/li>\n\n\n\n<li>Providers who want to offer COVID-19 vaccine to their eligible patients.<\/li>\n\n\n\n<li>Providers that can estimate the number of uninsured or underinsured adults their site currently sees or will see. Note: This information will be required on the Bridge Program enrollment form. Sites will not be able to enroll without this information.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading has-text-color has-medium-font-size\" style=\"color:#0f4d90;font-style:normal;font-weight:700\"><strong>Who counts as uninsured or underinsured?<\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Adults not covered by any health insurance plan. &nbsp;&nbsp;<\/li>\n\n\n\n<li>Adults who have health insurance, but coverage does not include any vaccines.<\/li>\n\n\n\n<li>Adults who have health insurance, but coverage does not include all vaccines recommended by the Advisory Committee on Immunization Practices (ACIP).<\/li>\n\n\n\n<li>Adults who have health insurance, but there is a fixed dollar limit or cap for vaccines.<\/li>\n<\/ul>\n\n\n\n<div class=\"wp-block-buttons is-content-justification-center is-layout-flex wp-container-core-buttons-is-layout-fe48e5de wp-block-buttons-is-layout-flex\">\n<div class=\"wp-block-button\"><a class=\"wp-block-button__link has-background wp-element-button\" href=\"https:\/\/forms.monday.com\/forms\/4b7ebe053918f907003da3fdabfb1871?r=use1\" style=\"background-color:#ff7474\">Bridge Program Interest Form<\/a><\/div>\n<\/div>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The Immunization Program expects to launch the Bridge Program in early fall, with an exact launch date still forthcoming, but in alignment with commercialization of COVID-19 vaccine.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">To gain a better understanding of the numbers of sites interested in participating in the Bridge Program, we ask you to complete the above survey by Friday, July 14, 2023.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Interested in the COVID-19 Bridge Program?
