Clinics enrolled in both programs must complete separate re-enrollments for each program.<\/strong><\/p>\n\n\n\n<p><\/p>\n\n\n\n<p style=\"font-size:24px\"><strong>Complete re-enrollment in 3 steps:<\/strong><\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><strong>1. Submit the&nbsp;required&nbsp;survey<\/strong><\/p>\n\n\n\n<p><\/p>\n\n\n\n<p>This submission is&nbsp;required&nbsp;to complete re-enrollment and helps us better support your clinic. Update your Clinic Tools information<\/strong><\/p>\n\n\n\n<p><\/p>\n\n\n\n<p>Update the information indicated in the&nbsp;&#8216;Clinic Tools, Clinic Information&#8217; module<strong>&nbsp;<\/strong>for your clinic. Use these guides to make the&nbsp;necessary&nbsp;changes to the Clinic Tools page:<\/p>\n\n\n\n<p><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Staff changes:<strong>\u00a0<\/strong><u><a href=\"\/index.php\/portfolio_page\/staff-changes-job-aid\/\" target=\"_blank\" rel=\"noreferrer noopener\">Staff Changes Job Aid<\/a><\/u><\/li>\n\n\n\n<li>Vaccine coordinator trainings:\u00a0The primary and back-up vaccine coordinators must document completion of the <a href=\"https:\/\/www2.cdc.gov\/nip\/isd\/ycts\/mod1\/courses\/vfc\/ce.asp\">CDC&#8217;s\u00a0You Call the Shots training<\/a>.\n<ul class=\"wp-block-list\">\n<li>VFC:\u00a0\u00a0If you have taken this training before, you may complete the\u00a0<u><a href=\"https:\/\/www2.cdc.gov\/vaccines\/ed\/vfc\/vfc\/\" target=\"_blank\" rel=\"noreferrer noopener\">VFC Refresher Test<\/a><\/u>\u00a0instead.<\/li>\n\n\n\n<li>VFAAR only:\u00a0If you have taken this training before, you may complete the\u00a0<u><a href=\"https:\/\/www2.cdc.gov\/vaccines\/ed\/vfc\/sh\/\" target=\"_blank\" rel=\"noreferrer noopener\">Storage and Handling Refresher Test<\/a><\/u>\u00a0instead.<\/li>\n\n\n\n<li>Add the training(s) to your PhilaVax staff page by following <u><a href=\"https:\/\/www.youtube.com\/watch?v=5_IFE7pIFH8\" target=\"_blank\" rel=\"noreferrer noopener\">these instructions<\/a><\/u>.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Address and delivery hours:\u00a0<u><a href=\"\/index.php\/portfolio_page\/clinic-address-delivery-hour-update-guide\/\" data-type=\"link\" data-id=\"\/index.php\/portfolio_page\/clinic-address-delivery-hour-update-guide\/\" target=\"_blank\" rel=\"noreferrer noopener\">Address or Delivery Hours Update Guide<\/a><\/u><\/li>\n<\/ul>\n\n\n\n<p><\/p>\n\n\n\n<p><strong>3. Complete the enrollment form in PhilaVax<\/strong><\/p>\n\n\n\n<p><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Be prepared to:\n<ul class=\"wp-block-list\">\n<li>Enter updated patient population data.\n<ul class=\"wp-block-list\">\n<li>Use this guide:\u00a0<u><a href=\"\/index.php\/portfolio_page\/vfc-vfaar-enrollment-job-aid\/\" target=\"_blank\" rel=\"noreferrer noopener\">Enrollment Guide<\/a><\/u><\/li>\n\n\n\n<li>When completing the enrollment, you will be asked to provide updated patient population data.
